Please note that the following items must be referenced on all insurance declaration pages:

1. All persons occupying the apartment, over the age of 18, must be listed on the policy OR they must provide us with separate insurance coverage.

2. List "Apartment Community Name-Richdale Apartments" as additional insured with the following address: PO Box 115009, Carrollton, TX 75011 as "Additional Insured" or "Interested Party".

3. Minimum liability coverage of $100,000 is required.

4. Policy number and effective dates.

5. List your “insured address” including your apartment street address and unit number.
